What information we collect

We collect personal information necessary to provide NDIS supports, including your name, contact details, date of birth, NDIS number, disability and health information, support needs and goals, and information from referrals and assessments. We only collect information that is reasonably necessary for the supports we provide.

How we use it

We use your personal information to deliver, coordinate and improve the NDIS supports you receive, to meet our obligations as a registered NDIS provider, to communicate with you and your support network, and to comply with legal and regulatory requirements including reporting to the NDIS Quality and Safeguards Commission.

Who we share it with

We may share your personal information with other service providers involved in your care, the NDIA and the NDIS Quality and Safeguards Commission, government agencies where required by law, and other parties with your consent. We do not sell or rent personal information to third parties.

How we store it

Personal information is stored securely in electronic and physical systems with access restricted to authorised staff. We take reasonable steps to protect your information from misuse, interference, loss, unauthorised access, modification or disclosure.
